| The card is accepted at most major pharmacy chains. In addition, many independent pharmacies may also honor the Discount Card. Use the pharmacy locator or contact our Customer Service at 1-888-______-_________ (INSERT NUMBER) to obtain pharmacy location information. If a neighborhood pharmacy is not already participating in our network, please have them call us. The pharmacy will be sent information on how to become a participating provider. |

| What if a pharmacy listed as participating does not recognize the card? |
| If the pharmacist does not recognize your card or if you encounter a problem at a participating pharmacy, have the pharmacy call the toll-free number printed on the Discount Card. |

| How much will
I pay for my prescriptions? |
| There are four pricing levels for prescriptions. A Discount Program member pays $10 or less for prescriptions in Level 1, prescriptions in Level 2 are $20 or less and in Level 3 prescriptions are $40 or less. All prescriptions with discounted prices greater than $40 are categorized in Level 4. Prices and corresponding prescription levels on this website reflect average prescription prices with the Discount Card. Prices will vary depending on quantity, pharmacy, and your geographic location. Contact a local participating pharmacy for the exact price of your prescription with the Discount Card. | There is no guaranteed percentage of savings on any drug. In order to compete in the market, some pharmacies offer a cash price that is lower than the discounted rate for certain prescriptions. | What is a generic
drug? |
| Following the Food and Drug Administration’s (FDA) approval of a new drug for sale in the United States, the manufacturer is provided an exclusive period of time in which no other company may sell the same pharmaceutical. This exclusivity period allows the manufacturer to price the drug so that it may recover its investment in the research and development of the new drug. After this time has elapsed, other companies can manufacture generic versions of the brand medication. Since the production of generic medication does not require a large investment in research, development and advertising, the cost of a generic is significantly less than that of brand medication. Generally, the cost of a generic medication is about half the cost of a comparable brand product. Regardless of whether drugs are brand name or generic, all prescriptions must meet the same FDA standards of quality. |

| We encourage all cardholders to use generic drugs whenever they
are available. Using generic drugs is an important way that you
can help to save money for yourself. Generic drugs contain the
same active ingredient(s) found in the brand-name product. All
generic drugs must meet the same FDA standards of quality as the
brand-name drug. |
